What to include in your referral

To speed up intake, include as much of the following as possible. Nothing is a blocker to getting started.

  • Participant initials (for privacy before Service Agreement)
  • Current NDIS plan — relevant funding categories
  • Service type required (SIL, Support Coordination, etc.)
  • Preferred location / suburb
  • Your contact details (phone and email)
  • Any urgency factors or time-sensitive needs
  • OT Functional Capacity Assessment (optional but speeds matching)
  • Handover or progress reports from previous providers

How quickly will you respond to my referral?
We acknowledge all referrals within 24 hours (one business day). You receive an automated confirmation with your referral ID immediately on submission, followed by a personal response from our intake coordinator.
Do I need all the documents before I submit?
No. The NDIS Plan is required so we can confirm funding, but the FCA, handover report, and behaviour plans are optional. You can upload additional documents after submission via your Disability OS tracking link. Nothing blocks getting started.
Is my participant's information kept private?
Yes. The form only requests participant initials — protecting privacy before a Service Agreement is signed. All data is transmitted over 256-bit encryption and stored in compliance with the Australian Privacy Act 1988.
What is Disability OS?
Disability OS is our proprietary intake and case management platform. Once submitted, you receive a live tracking link to monitor intake status, upload documents, and message your GenCare intake coordinator in real time from any device.
Can I refer multiple participants at once?
Yes. Complete a separate referral for each participant. All referrals appear on your Disability OS dashboard so you can track multiple intakes simultaneously.
What services can I refer into?
GenCare accepts referrals for Supported Independent Living (SIL), Specialist Disability Accommodation (SDA), Community Participation, Support Coordination, and Psychosocial Recovery Coaching. Call 1300-247-788 if you're unsure which service fits.
